If FD Form 249 was previously prepared and forwarded to the FBI, a disposition sheet Form R-84
(FBI) specifying the charge, sentence, and FBI file number will be completed and forwarded to the
FBI.
If FD Form 249 was not prepared and forwarded to the FBI, one will be prepared showing the
charge and sentence and forwarded to the FBI.
(2) Completed FD Form 249 and R-84 (FBI) will be forwarded directly without letter of transmittal to
United States Department of Justice, Federal Bureau of Investigation, ATTN: Identification Division,
Washington, DC 20537. A supply of FD Forms 249 and Forms R-84 (FBI) may be obtained through normal
publication supply channels.
